In order to make your visit more convenient for you, we have designed this website so you may download and print the new patient forms to be completed prior to arriving at our office.
PLEASE READ BEFORE PRINTING :
- Select from the two buttons: Auto Accident or Cash/Insurance.
- Print out the forms.
- Complete the forms with as much detail as possible. As you can see, I like to be thorough with my patients and the more detail I have regarding your condition and your overall health, the better chance you will have of obtaining optimum results with your care.
- Make sure you bring the completed forms when you come for your first visit. We have scheduled your appointment knowing you have the forms. If you arrive without the forms, it will take extra time for you to complete them and we may not have the time to give you a proper consultation and examination.
If you have already had testing at another office such as x-rays, MRI's, CAT scans, or EMG's, please bring the reports with you if possible. Also, if you had x-rays or an MRI, it is very helpful if you bring a copy of them with you so Dr. Wray may review them.
